EMPLOYEE MISCONDUCT REPORTING FORM
Employees of the District shall not commit or participate in misconduct. This may include:
  • Waste or misuse of public funds, property, or manpower
  • Violations of a local, state, or federal law and/or regulation applicable to the District
  • Violations of District policy or rule
  • Gross mismanagement
  • Abuse of authority
  • Unethical conduct

  • This form is for employees of Weber Basin Water Conservancy District to report misconduct. Each submission will be taken seriously and fully investigated. Employees are protected by Utah and Federal whistleblower laws, including Utah Code Title 67, Chapter 21 and Utah Code Section 34A-6-203. Anonymous reports may be dismissed and may not receive whistleblower protections.
    Please be aware that the District may be unable to investigate an anonymous complaint. The more information provided, the better.
