EMPLOYEE MISCONDUCT REPORTING FORM

Employees of the District shall not commit or participate in misconduct. This may include:

Waste or misuse of public funds, property, or manpower

Violations of a local, state, or federal law and/or regulation applicable to the District

Violations of District policy or rule

Gross mismanagement

Abuse of authority

Unethical conduct

This form is for employees of Weber Basin Water Conservancy District to report misconduct. Each submission will be taken seriously and fully investigated. Employees are protected by Utah and Federal whistleblower laws, including Utah Code Title 67, Chapter 21 and Utah Code Section 34A-6-203. Anonymous reports may be dismissed and may not receive whistleblower protections.

1. What is the assertation of improper activity? Please describe in detail.

2. Who is the person(s) the complaint is against? Please provide name, position, and department.

3. When did the event(s) take place, please include dates, time, and frequency.

4. Where did the events occur?

5. Are there any persons who might provide information or who witnessed the event? Please provide names and contact information.

6. Is there evidence or documentation that can be reviewed? Please provide any available documentation to the District. Documentation can either be emailed, mailed, or hand delivered to the District.

7. How do you know about the improper action? Did you see it occur? Did you hear about it from someone else? Did you see documentation indicating it occurred?

Please be aware that the District may be unable to investigate an anonymous complaint. The more information provided, the better.
